Personalisation

At Avenues we want the person we support to be in control of their support needs.  There has recently been a huge move towards personalisation across all areas of social care, giving people who require social care support more choice and control in arranging for their own needs to be met.  Avenues provides a service that responds to the person we support instead of the person having to fit in with one of our services. 

Some of the ways that Avenues is responding to Personalisation include:

  • Planning the support to fit the persons individual needs through person centred planning;
  • Developing community mapping and partnerships in all the areas where we work to help people build up natural community connections and networks local to where they live, helping them to get involved in the community;
  • Supporting people to live in their own homes and helping them to develop their own independent living skills;
  • Providing knowledge of housing options including shared ownership to help people choose how and where they live;
  • Supporting the people we support to purchase their own support through direct payments;
  • Giving the people we support the opportunity to have a say in how the service they live in is run;
  • Supporting carers of young people to help them to maintain a life beyond their caring responsibilities;
  • Using assistive technology to enable people to maintain their independence;
  • Ensuring that our staff are trained in person centred active support so that the people we support are able to take part in meaningful everyday activities regardless of how much support they require.
