Home Care
The Avenues Trust Group provide home care support to older people, to people with disabilities - in fact to anyone who has difficulty in managing every day tasks, in their own homes, without some support.
We currently operate in the Medway area. We specialise in offering support to older people with challenging or specific needs, working with their carers to provide the best possible outcomes that supports people's choice to remain in their own homes.
- Enablement - we work with the people we support to assist them to regain lost skills, helping them to maintain their independence
- Night service - we provide a night-time call service, visiting people in their homes to assist with medication, comfort and dietary needs
We work closely with Local Authorities or alternatively with you individually if you are in receipt of a Direct Payment.

Dementia
The Dementia Support Service provides support to people with dementia, and their carers, in their own homes. Our employees are fully trained in dementia care and will work with the individual to provide support to maintain their independence and enjoyment of life.
We work one-to-one with people with dementia to support them to maintain their everyday skills and to undertake activities that they enjoy to enable them to live as independently as possible for as long as possible.
Our employees are skilled in memory reminiscence work and work sensitively with people to encourage conversational skills and to enable the person we are supporting to maintain their interests in life.
We can offer respite services to support carers to enable them to have a break and will provide the same experienced trained staff who, over a period of time, will build up a trusting relationship with the person we are supporting.
We understand that sometimes people can become frustrated and frightened and work at people's own pace to encourage and support them to do things for themselves.
Knowing the person in key to providing support to people with dementia and we will work with individuals to build up a detailed picture of them, their interests and their wishes, to support them to in the way that is right for them.
