CRB Enhanced Disclosures

All support staff will require an Enhanced Disclosure prior to commencing employment.
The Disclosure Application Form and guidance booklet for completion will, where possible be sent with the interview letter and the completed form and sufficient supporting documents will need to be bought to the interview.
Processing the disclosure application by the Criminal Records Bureau can take between 4 - 6 weeks therefore it is essential that the form is completed correctly and there are a sufficient number of supporting documents from the list below. Please note that if further documents/ information are required the process will be delayed further.
WHAT DOCUMENTS DO YOU NEED TO PROVIDE?
Can you provide a document from Group 1?
ALL DOCUMENTS MUST BE IN YOUR CURRENT NAME (EXCEPT MARRIAGE CERTIFICATE).
AT LEAST ONE DOCUMENT MUST SHOW YOUR CURRENT ADDRESS AND AT LEAST ONE DOCUMENT MUST SHOW YOUR DATE OF BIRTH.
VALID DOCUMENTS
Group 1
- Passport - any nationality
- UK Birth Certificate - issued within 12 months of date of birth - full or short form accepted included those issued by UK authorities overseas, such as Embassies, High Commissions and HM Forces
- UK issued Driving Licence - England/Wales/Scotland/Northern Ireland/Isle of Man; either photocard or paper. A photocard is only valid if presented with the counterpart licence
- EU National Identity Card - EU countries only
- HM Forces ID Card (UK)
- UK Firearms Licence
- Adoption Certificate (UK)
Group 2
- Marriage/Civil Partnership Certificate
- Financial Statement ** e.g. pension, endowment, ISA
- Birth Certificate
- Vehicle Registration Document (Document V5 old style and V5C new style only)
- P45/P60 Statement (UK) **
- Mail Order Catalogue Statement*
- Bank/Building Society Statement*
- Court Claim Form (UK) ** document issued by Court Services
- Utility Bill * electricity, gas, water, telephone - inc mobile phone contract/bill
- Exam Certificate e.g. GCSE, NVQ, O Level, Degree

- One of the following documents from the Borders and Immigration Agency (BIA) (formerly the Immigration and Nationality Directorate - IND) (UK) Do not use more than one of the following documents. Convention Travel Document (CTD), Stateless Person's Document (SPD), Certificate of Identity (CID), Application registration Card (ARC)
- Connexions Card (UK)
- CRB Disclosure Certificate **
- Letter from a Head Teacher *
* Documentation should be less than three months old

COMMON ERRORS
- Completed in the wrong colour ink - The form must be completed in BLACK ink
- Five year address history - Complete address history is required covering the past five years
- Middle name missing - all current forenames must be entered in section A and any previous forenames used must be entered in Section C along with the year used from and to.
- Surname at birth missing - If you cross MS or MRS in section A, your surname at birth must be entered in Section C even it is the same as the surname recorded in Section A. Any previous surnames used must be entered in Section C as well along with the year used from and to.
POINTS TO REMEMBER
- Certain fields are mandatory and must be completed. This will be fully detailed in the guidance booklet you receive.
- If you make a mistake on the form, make the correction to the right of the error. If there is no space to write next to a particular piece of information, please write as close to the text as possible.
- DON'T use correction fluid on the form. To correct a mistake, please follow the guidance above.
- DON'T allow text to cross the edges of the boxes on the application form.
- DON'T put a line through a section of the form or state a field as ‘not applicable'. If it is not relevant to the application then please leave it blank.
- Your contact numbers are not mandatory but can assist the Criminal Records Bureau if they require further information. Without your contact numbers, if further information is required, the form would have to be returned and can therefore delay your recruitment.
- If you have more than one previous address, please list these on a separate piece of paper.
